As the end of your pregnancy approaches you are probably becoming a little nervous about what to expect, how you will know you are in labor, and when the call the doctor. Every new mother has these same fears and worries. Knowing how to identify “real” labor, having all the phone numbers handy of who to call when your labor starts, and talking to your doctor about your fears will all help to ease your worries.
So what is labor? Labor is what your body does in order to prepare for and deliver your baby. The contractions help to thin and open the cervix so your baby can move down and out of the birth canal.
Labor will generally start around your due date. If you are experiencing labor symptoms early call your doctor right away. Each pregnancy is different, some women have quick labors, while others will labor for much longer. Just like with everything else about pregnancy every one is different.
Your body will begin doing things to prepare for the labor and delivery any where from a few hours to a few days before your actual labor will begin. This is commonly known as pre-labor. Here are a few pre-labor symptoms to watch out for.
- Your may notice that your baby seems to have “dropped”. Some time during the last few weeks or days of your pregnancy the baby will begin moving into position for delivery. This is known as enlightening or "dropping".
- You may notice that your Braxton Hicks or false labor pains are becoming stronger. These are not actual labor pains but they do work to help begin thinning out the cervix in preparation for the real thing.
- Many women experience an upset stomach, vomiting and diarrhea in the days leading up to labor. Your body is getting ready for the pushing that will take place by cleaning out your system.
- You may experience bloody show (a blood streaked mucus). This typically happens as the cervix begins to dilate. The bloody show is also know as a mucus plug, which plugs the cervix during pregnancy to help prevent infections and bacteria from entering the uterus during pregnancy.
Given all the symptoms that your body is going through to prepare for “real” labor how will you know when the real thing has started? It can be difficult at first to know if what you are experiencing is indeed labor, in no time you will figure it out. Here are the signs that “real” labor has begun and it is time to call the doctor and head to the hospital.
- Contractions are coming with some frequency at regular intervals, don’t subside when you change position or drink water. When you begin to feel contractions you will want to begin timing them as the doctor will want to know how far apart they are and how long they are lasting.
- Unlike with false labor pains (Braxton Hicks contractions) you will feel real labor contractions in your back as well as your stomach. The contractions will typically begin in your back and move around to the front.
- You may feel abdominal cramping or an upset stomach, as well as experience diarrhea. This is thought to be caused by the increased hormones that prompt labor to begin.
- Your water will break, also known as ruptured membranes. Not all women will experience this, some women will have to have their water broken after arriving at the hospital.
If you experience any of these symptoms, or you have other indications that labor has begun contact your doctor. Your doctor will likely get some information from you to try to determine if this is the real thing and how far along you are. If this is indeed the real thing the doctor will advise you to head to the hospital.
Knowing what to expect and how to identify the symptoms of labor can be a little nerve wracking for new mothers. Talk to your doctor or midwife about your concerns and fears. They can help educate you so you feel better prepared.
Here are a few helpful websites.

Courtney,
You should talk with your doctor or midwife about your concerns. My website is designed to provide information, but I am not a doctor and cannot provide health care advise.

i am 36 weeks pregnant, already had labor twice causing me to dilate 2cm and become thinned 70%. now all of what i experienced was really strong prelabor which was stopped. now, i have continued having these prelabor pains and they are still strong that sometimes i cry from the pain. but i know they arent real. what should i do b/c i know i can go anytime. i know that they wont be stopped once the real stuff comes, my son has had the development shots already and his weight is good. i am fearful that my water wont break and it will be the real thing but b/c i have dealt with what i have dealt with and it feels real that i wont know the difference? and what can i do for these cramps, they are the tearjerkers as i feel like im about to start my period and its consistant when it hits.

I am almost 37 weeks pregnant and noticed that my breasts are getting “ready”… starting to get fuller and more tender, it seems as if my body is producing colustrum? Does that mean a certain time frame that I will be going into labor soon? Or is this just normal for the end of pregnancy?
September 22nd, 2007 at 3:40 pm
Amanda,
I know that most women start to produce colostrum towards the end of their pregnancies. As far as if that means labor will start soon, I don’t think so, but I would ask your doctor to be sure.

Sarah,
I would recommend calling your OB and find out what they think about the symptoms you are having. It never hurts to get checked out.

I am 39 weeks and my baby moves a lot more than before and sometimes is painful why is that?
February 19th, 2008 at 9:17 am
Maria,
Babies do tend to move a lot when you are about to go into labor. I would talk to your doctor about it, maybe they can check you to see if you have started dilating.

It’s possible. I would call your doctor and run your symptoms by him/her to see what they think.

I’M 39 WEEKS AND HAVING CONTRACTIONS IN MY BACK AND CRAMPS IN MY LOWER FRONT SHE MOVES A WHOLE LOT I’VE BEEN DILATED TO A 3 AND 80% EFACED FOR TWO WEEKS DOES THIS MEAN I’M GETTING CLOSE? SHE’S SO LOW IT HURTS TO SIT AND BEND TO DO ANYTHING.I ALSO HAVE SHARP STABBING PAINS IN MY VAGINA WHAT IS THAT?PLEASE ANSWER ME BACK!!
May 20th, 2008 at 7:18 pm
Brandy,
I don’t know what those stabbing pains are. It is always best to run your concerns past your doctor just in case there is something wrong. I know many women can walk around dilated and effaced for several weeks. Until those “real” contractions start or your water breaks there is no way to know when labor will begin.

You should definitely call your doctor to explain the symptoms you are having and find out if he/she wants to see you or have you go to the hospital.

Hi. I am currently 37 weeks pregnant. And for almost a week now I have been experiencing False labour pains. Everything from back pain to sharp pains down my groine area. Also in the last few days my babies activity has slowed down. Could this mean I am about to go into labour?
July 14th, 2008 at 1:39 pm
Hi Tami,
Congratulations. Those are some of the signs that you may go into labor soon, however, false labor pains can last for weeks. I would recommend calling your doctor and describing what you are experiencing and see what he/she thinks.

Lovely site, Lisa. I begin week 38 day after tomorrow. I feel completely well and am just counting off days till baby decides to debut. I hope to have a natural delivery and need to know whether a painless delivery is really recommended.
July 16th, 2008 at 7:01 am
Natasha,
Thanks, I’m glad you like the site. Best of luck with your delivery. As far as natural or medicated delivery, it is completely up to the woman. My recommendation is to go into it with your plan but know that things may change and to be open to changing your mind. Hope all goes well for you.

I’m 36 weeks pregnant and I have so much pain in my hips that I can’t even walk. Is this just pressure from the baby moving down or could this just be another early labor sign? I’ve been having contractions (braxton) for almost 4 weeks now. I hope this is over soon as the pain is so great I think something is wrong.
July 29th, 2008 at 6:49 am
Brenda,
As the baby moves into position and your body prepares for delivery it does cause some pain in your hips and pubic bone area. However, I would recommend talking to your doctor about the pains you are having just to be safe.

I am 35 weeks pregnant and have started experiencing pains similar to round ligament pain in my vaginal area (only on the left side). It happens when I get out from sitting or lying down. I have to hold that area because the pains can be severe. Any idea what is going on?
August 17th, 2008 at 3:35 pm
Renae,
I’m not sure. I would mention it to your doctor, maybe he/she will know what it might be.

Ashling,
I would start by saying you should mention your symptoms to your doctor. I had a similar experience in my own pregnancy and it was when the baby dropped a bit during the last couple of months of pregnancy. Many women experience the same thing. The baby drops causing more discomfort in the back and groin, puts more pressure on your bladder, etc. Also, your pelvic bones begin to separate in preparation for delivery. But definitely mention it to your doctor just to be safe. Best of luck to you.

August 20th, 2008 at 12:21 pm
Michelle,
Many women experience those same things as the baby drops and gets ready for delivery. I would definitely mention anything you experience to your doctor so they can check you and make sure everything is ok.

Hey ladies! I am 37 weeks and have already had my “nesting” spurt (washed the pans under my burners etc.) and am now having more intense Braxton-Hicks and more period-like cramping. I have had to wear panty liners for several weeks now, too; fun stuff, that mucus. Hoping this means soon, but not holding my breath lol. Just wanted to share my take on those “stabbing my vagina” pain comments - I get those a lot, and with my first pregnancy, too. I notice them when my babies get their arms moving, and have always attributed them to having my cervix assaulted, as it feels about the same as when my husband get a little, um, frisky, its just coming from the inside rather than the out! Good luck to all!
August 24th, 2008 at 3:18 pm
Andrea,
Thanks for sharing! Ah, nesting. My house was never so clean and organized as the week before my daughter was born
Good luck to you, you’re almost there now!

I’m 38 weeks pregnant. About 2 days ago I felt increased nausea and in fact was up all night throwing up. I had morning sickness for a long time, but that hasn’t been an issue for a while now. I also have really watery diarrhea and gas. I’ve been to the hospital to get fluids, but is my vomitting and diarrhea a bug I picked up somewhere(no one i know has been sick lately…) or am I going into labor? I have also felt tightening in my stomach the past few days, but becuase I’ve been feeling so sick I haven’t been very active.
August 25th, 2008 at 7:32 pm
Oh, and at the hospital the nurse said that I’m only 1cm+ dialated, but 80% effaced and that the head was really low.
August 26th, 2008 at 7:27 am
Andrea H,
Some women do experience diarrhea a few days leading up to labor. However, I would run your symptoms by your doctor and see what they think.
